2012 – 2013 scholar applications are due April 1, 2012.  Applicants must meet the following criteria in order to apply:

• Residency within one of the following zip codes: 60618, 60622, 60634, 60639, 60641, 60647 and 60651.
• Applying for admission to an all-girls Catholic high school as a freshman or returning Madonna Scholar.
• Demonstrated family financial need.
• Demonstrated scholastic achievement, effort or potential.
• A satisfactory letter of recommendation.
• Receipt of signed Statement of Commitment and Cooperation from parent or guardian.
• A commitment to attend and participate in Madonna Scholars’ programming throughout the year including forums, workshops, social events and spiritual retreats.
• A commitment to continuous good behavior and attendance.
• A commitment to completing a mimimum of five hours each of involvement in extra-curricular activities and community service per semester.
